The BC Children’s Hospital Care Hub helps pediatric T1D patients and their families work together to better manage diabetes using simple and secure digital tools. This app is currently being used as part of the BC Children’s T1D TrustSphere Pilot Study.
Patients and their parents can link, sync & view data from their diabetes devices (glucose sensors, etc), use Careteam to connect with their healthcare team, and choose to share data to research. Families will also find recommended tools and resources like educational material that will help them as they navigate diabetes on a day-to-day basis.
The BC Children’s Hospital Care Hub is available thanks to a collaboration between the BC Children’s Hospital Research Institute, Careteam, IDENTOS, Smile CDR, and Secure Key.
